MEDICATIONS

- Prescription and/or Over-the-Counter Medications will NOT be dispensed by the School Nurse unless a doctor's written order is submitted to the health office each school year.
- Medication orders along with the prescribed Medication should be bought to the health office by a Parent/Guardian
- Medication can NOT be kept in the health office unless it is received with the proper completed medication order
- DO NOT send medication into school with a student
- Please check the expiration date to ensure the medication is not expired
- Student's may NOT carry or use medication in school independently without a self-carry doctor's order on file. (ie. Tylenol, Inhaler)
- Medication Forms are available in the health office or can be downloaded from this site and the WFSD web site.
- Medications not picked on the designated day(s) at the end of the year will be DISCARDED.
- Parents/Guardians may come up to the school to administer medication to students who do not have MD orders on file.
ASTHMATICS
- Tubing/Masks for Nebulizer Medications must be provided by the parent/guardian
- Students may NOT self-carry an inhaler without a doctors order on file for the present school year
FOOD/VENOM ALLERGIES
- An Allergy Action Plan Form must be completed by your health care provider if an Epi-pen is required
- Students may NOT self-carry an Epi-pen without a doctors order on file for the present school year

DIABETICS
Must Have All of the Following Available in School:

- Doctors orders; including Insulin Coverage Formula and Insulin to Carbohydrate Ratio
- Glucagon Injection Kit
- Juice and/or Snacks
- Fast acting Glucose tabs and/or gel
- Glucometer (test strips, lancets)
- Ketostix
- Insulin (pod, pump, pen)
SEIZURE DISORDER: with DIASTAT Order
- Contact the health office immediately when you receive notification of any up coming filed trips and/or activities
- Student MUST have a designated Adult attend ALL field trips OR a nurse has to be assigned to the trip (the student will be placed with the nurse's group and bus)
- Prior notification of no less than 1 week is required when a student plans to stay after school for any reason ie. extra help * A parent or nurse MUST be available
- Doctor's orders MAY indicate that, "Diastat is NOT required for Field Trips, Sports and/or After School Activities: Call 911 for any seizure activity." this order allows for NO prior notification when a student plans to stay after school or play sports. This order avoids the student having to be placed with the nurse on school trips
- 911 is called if DIASTAT is administered
