- Take the time to check with your health care provider TODAY and schedule an appointment if needed.
- Appointment dates must be BEFORE the first day of school if your child has not yet received his/her required vaccination(s). (Call today to guarantee your appointment will be before the first day of school to avoid exclusion)
- 6th grade Tdap: Students will be excluded from school attendance after the permitted 14 day grace period from their 11th birthday; be sure and schedule your child's appointment for before that date, to receive their Tdap.
- Contact your school nurse today to ask which immunization(s) may be required for the up coming school year.
- Always provide the school nurse with updated immunization records.
- Medical Exemption Forms must be completed every school year and submitted prior to the first day of school.
- Religious Exemption Forms must be completed and received by district office prior to the first day of school.

IMMUNIZATION REQUIREMENTS for 6th, 7th and 8th graders to attend school:
Proof of the above immunizations must be provided in any 1 of these 3 ways:
- 2 Varicella (chickenpox)
- 3-4 Polio (if 3 were given, one must be dated after the age of 4 years old or a 4th dose is required)
- 1 Tdap (Not required until age 11)
- In ADDITION for ALL 7th & 8th graders: 1 Meningococcal MCV4 (meningitis) to attend school
Proof of the above immunizations must be provided in any 1 of these 3 ways:
- An immunization certificate signed and stamped by your health care provider
- Immunization Registry report (NYSIIS or CIR from NYC) from your health care provider or any county health department
- A blood test (titer) lab report that proves your child is immune to measles, mumps, rubella, varicella, hepatitis B and/or polio
Request that your health care provider input your child's immunizations into the New York State Immunization Information System (NYSIIS) so the school health office has easy access to updated immunization records.
* Students in Non-Compliance with be EXCLUDED FROM SCHOOL until proof is received
